- Brushing does not reach between teeth. A brush cleans the outer and inner surfaces well, and the contact points between teeth poorly. - Minerals in your saliva harden what is left. Calcium and phosphate from saliva crystallize inside the plaque film. The places a toothbrush almost never wins
Some spots collect tartar faster than the rest of the mouth, and they are the same in nearly every patient:
- Behind the lower front teeth, where the sublingual salivary ducts open and bathe the area in mineral-rich saliva
- The cheek side of the upper back molars, opposite the parotid duct, for the same reason
- Between crowded or overlapping teeth, where the brush physically cannot fit

- It has been six months or more since your last clean
- You smoke, chew paan or gutka, or drink a lot of tea
- You wear braces, a bonded retainer, a partial denture, or have implants
- You are pregnant, diabetic, or have heart disease, where gum inflammation matters more
It is not the right treatment, or not the whole treatment, when:
- The problem is intrinsic tooth color rather than surface stain, which cleaning will not change
- There is active decay or a broken tooth needing attention first
- Deep pockets and bone loss are present, where a simple clean is not enough and root planing or periodontal care is needed

- Clean between the teeth daily, with interdental brushes where they fit; a Cochrane review of 35 trials concluded that floss or interdental brushes used alongside brushing may reduce gingivitis and plaque more than brushing alone, with interdental brushes possibly the better of the two, though it rated the evidence low certainty (Cochrane Database of Systematic Reviews, 2019)
- Spend extra time behind the lower front teeth, since that is where deposit forms first
- Rinse with water after tea, paan, or sweetened drinks
- Stop smoking and chewing tobacco, which speed up both deposit and bone loss
