If you have ever run your tongue along the back of your lower front teeth and felt a rough ledge that will not brush away, you have already met hardened tartar. Scaling and polishing in Islamabad is the only reliable way to remove it, and the reason is simple: by the time you can feel it, it is no longer soft deposit. Why brushing stops working after about two weeks

The four stages, and which ones you can still undo

  1. Soft plaque, hours to days. Barely visible, and removable at home. Fully reversible.
  2. Early calculus above the gum, around two weeks. A chalky or yellow ledge, usually behind the lower front teeth and on the outer surfaces of the upper molars. Needs scaling, but the gums recover completely.
  3. Calculus below the gum line, months. Deposits creep under the gum margin, where the body treats them as a chronic irritant. Gums bleed and a pocket starts to form. Scaling still works, but now it has to reach below the gum.
  4. Bone loss, years. The support around the tooth recedes. This is the line that matters: gum inflammation is reversible, lost bone is not. Treatment at this stage aims to halt progression and save what remains.

Nearly everyone we see is at stage two or three. Very few arrive at stage one, and the ones at stage four usually came in for something else.

How to tell which stage you are at

A quick self-check, with the honest caveat that only an examination and often an X-ray can confirm it:

  • Pink in the sink when you spit. Not normal. Healthy gums do not bleed from brushing.
  • A rough ledge you can feel with your tongue, especially behind the lower front teeth.
  • Gums that look puffy or darker red at the margin rather than firm and pale pink.
  • Persistent bad breath that mouthwash only masks for an hour.
  • Teeth that look longer than they used to, which often means the gum has receded.
  • A tooth that feels slightly mobile. This one warrants an appointment soon rather than eventually.

Why it collects in the same three places every time

Tartar is not random. It builds where saliva ducts empty, because that is where the minerals are most concentrated:

  • Behind the lower front teeth, opposite the ducts under the tongue
  • On the cheek side of the upper back molars, opposite the ducts in the cheek
  • Along the gum line between crowded or overlapping teeth

If your dentist keeps finding deposits in the same spots, that is the explanation, and it is also where a few extra seconds of brushing pays for itself.

Scaling or deep cleaning: which will I need?

These are two different procedures and the gap in price reflects a real gap in the work:

  • Scaling and polishing. Ultrasonic removal of deposits above and just below the gum line, followed by a polish. Usually one appointment of 30 to 45 minutes. This is what stage two and most of stage three need.
  • Root planing, or deep cleaning. Where pockets have formed, the root surface itself is cleaned and smoothed below the gum, often under local anesthetic and usually split across more than one visit, working through the mouth in sections.

You should not be quoted for root planing without someone first measuring your pocket depths with a probe. If a deep clean is recommended and no one measured anything, ask why.

The gaps that appear afterward

This surprises people and occasionally upsets them, so it is worth saying before rather than after. Where heavy tartar has been sitting for a long time, it has been occupying space the gum used to fill. Remove it and you may see small dark triangles between the teeth.

The tartar was not holding your teeth together. It was hiding recession that had already happened. The spaces were there before the appointment; you simply could not see them. Gums often tighten over the following weeks and reduce the effect.

What scaling will not do

Honest limits matter more than a long service list:

  • It will not whiten your teeth. It removes the stained deposit and reveals your actual tooth color, which is often lighter, but it does not change the shade of the enamel itself.
  • It will not regrow lost bone or gum.
  • It will not fix decay. Cavities need fillings.
  • It will not permanently cure bad breath if the cause is a dry mouth, sinus drainage or stomach reflux.
  • It will not stop recurrence on its own. Deposits start reforming the same week.

No. Ultrasonic scalers work at a frequency tuned to fracture mineral deposits, and the tip is designed to glance off enamel rather than cut it. The sensitivity some people feel afterward comes from newly exposed root surfaces that the tartar had been covering, not from damage to the tooth.

Why do my teeth feel loose after a cleaning?

Because heavy tartar can act like a splint between teeth. Removing it lets each tooth move independently again, which can feel alarming for a few days. If the looseness persists beyond a couple of weeks, it usually indicates existing bone loss and should be reviewed.

Is scaling painful?
