Pain at the Back of Your Mouth?

Wisdom Tooth Removal in Faisalabad: Protect Nearby Teeth From Crowding

A dull ache behind your last molar that comes and goes for months is the most common reason people start looking into wisdom tooth removal in Faisalabad. The worry underneath it is usually not the wisdom tooth itself. It is the tooth in front of it, the one you actually chew with, and whether the pressure from behind is slowly ruining it. That is a reasonable worry, and the answer is more specific than most clinic pages admit: a wisdom tooth does threaten its immediate neighbor, but not in the way most people have been told.

Signs Your Wisdom Tooth Is Already Affecting the Tooth Next Door

These are the things to look for, and they matter more than whether the wisdom tooth itself hurts:

  • Food packing between the last two teeth every single meal, in the same spot
  • A dull ache on one side that settles for weeks and then returns
  • Gum at the very back that is swollen, tender, or bleeds when you brush there
  • A bad taste or smell coming from the back of the mouth that returns within an hour of brushing
  • Pain when you bite down on that side specifically
  • Difficulty opening your jaw fully, especially in the morning
  • Sensitivity on the back of the second molar, the tooth in front of the wisdom tooth

If the bad taste comes with visible hard deposit around the back teeth, the first thing to rule out is build-up rather than the wisdom tooth itself. Our guide to scaling and polishing in Faisalabad covers that. If the pain is sharp on biting and lingers afterward, the nerve may be involved, which is a different problem explained in our guide to root canal treatment.

Does a Wisdom Tooth Really Crowd Your Front Teeth?

Probably not. A Cochrane review comparing removal with retention of asymptomatic impacted wisdom teeth found no evidence of a difference in late lower incisor crowding at five years. Lower front teeth do crowd with age, but removing a wisdom tooth is not a reliable way to prevent it.

This matters because "they will push your front teeth crooked" is the most common reason patients are given for removal, and it is the one the evidence supports least.

The Damage That Is Actually Well Documented

The real casualty is not your front teeth. It is the second molar, sitting directly against the wisdom tooth.

When a wisdom tooth erupts at an angle, it creates a space between the two teeth that no brush reaches and no floss passes cleanly. What collects there sits against the back surface of a tooth you need. Two things follow:

  • Decay on the back of the second molar. Often found only on an X-ray, and often too far back to restore well.
  • Gum disease around the second molar. The same Cochrane review reports very low-certainty evidence that retained asymptomatic impacted wisdom teeth may be associated with increased risk of periodontitis affecting that neighbor in the long term.

So the honest framing is this: the wisdom tooth is not crowding your smile. It is creating a trap against the tooth behind your smile, and that tooth is worth more than the one causing the problem.

Why the Angle of the Tooth Decides Everything

Not all impactions behave the same way, and the angle is what your dentist is reading on the X-ray:

  • Mesial angulation, tilted forward into the second molar. The most common, and the one that creates the food trap.
  • Vertical, upright but stuck below the gum. Often causes recurring gum infection over the top.
  • Horizontal, lying on its side against the neighbor's root. The highest risk to the second molar.
  • Distal, tilted backward. Frequently the least troublesome and sometimes left alone.

A tooth that is fully erupted, upright and cleanable may need nothing at all.

When Keeping It Is the Better Decision

A clinic that recommends removing all four as a matter of routine is not following the evidence. Overall, that same review concluded there is insufficient evidence to support or refute routine removal of wisdom teeth that are symptom-free and disease-free. Keeping yours is often right when:

  • The tooth is fully erupted, upright, and you can clean around it
  • There is no decay on it or on the second molar
  • The gum around it is healthy and has never flared up
  • You are able to attend regular check-ups so it can be watched
  • It opposes another tooth and is doing useful work when you chew

Removal becomes the clear answer when there is recurring infection, decay in either tooth, a cyst on the X-ray, or damage already visible on the second molar.

What the X-ray Shows Before Anything Is Decided

An examination alone cannot answer this question, and you should be wary of a plan made without imaging. A panoramic X-ray shows the angle of the tooth, how deep it sits in bone, whether the roots are curved or fused, whether decay has already started on the neighbor, and critically, how close the roots run to the nerve in the lower jaw. That last one changes both the technique and the risks your dentist will discuss with you.

Your Appointment, Start to Finish

  1. Assessment and X-ray. The angle, the roots, and the condition of the second molar are reviewed with you on screen.
  2. Plan and quote. Whether it is a simple or surgical removal, and what that means for cost and recovery, in writing before you commit.
  3. Local anesthetic. The area is fully numbed. You feel firm pressure during the procedure, not sharpness.
  4. Removal. A simple extraction takes a few minutes. A surgical one involves a small incision, sometimes dividing the tooth to bring it out in pieces, which spares surrounding bone.
  5. Closure. Stitches where needed, usually dissolving, and a gauze pack to bite on.

Our dentists at the People's Colony clinic are PMDC-registered, and every surgical case is planned from imaging rather than from the chair.

The First 48 Hours

This is the window that decides how smooth the rest of the week is:

  • Bite firmly on the gauze for a full hour without checking it repeatedly
  • Do not rinse, spit forcefully, or use a straw for 24 hours
  • Use a cold compress on the cheek for the first day, 15 minutes on and 15 off
  • Eat cool, soft food and chew on the other side
  • Take the painkillers before the anesthetic wears off rather than after
  • No smoking and no naswar, which considerably raise the risk of a dry socket

Swelling usually peaks around day two and then improves. Pain that worsens from day three onward, rather than improving, is the pattern that warrants a call.

How Much Does Wisdom Tooth Removal Cost in Faisalabad?

At DentoCorrect, a straightforward extraction starts from PKR 8,000 and a surgical removal from PKR 28,000, with the consultation free. Which one applies depends on the angle and depth of the tooth, so the figure is confirmed only after the X-ray. Published prices across Pakistan commonly span roughly PKR 3,000 to PKR 20,000 depending on complexity.

The spread exists because "extraction" covers two very different procedures. A fully erupted wisdom tooth lifted out in one piece is not comparable to a horizontally impacted tooth sectioned out of bone under surgical access. When comparing quotes, ask which of the two you are being priced for, and whether the X-ray, anesthetic and review visit are included.

Frequently Asked Questions

Can a wisdom tooth push my front teeth out of line?

The evidence does not support it. A Cochrane review found no difference in lower front crowding between patients who had wisdom teeth removed and those who kept them. Front teeth crowd with age regardless. If your concern is the appearance of your front teeth, orthodontic assessment is the relevant route, not extraction.

Can I keep a wisdom tooth that has never hurt?

Often yes. If it is upright, fully erupted, cleanable, and neither it nor the tooth in front shows decay, monitoring is a legitimate plan. What makes this work is attending check-ups so a problem is caught early. Removal is not automatically the safer choice.

Does removing a wisdom tooth change the shape of my face?

No. Wisdom teeth sit well inside the jaw and contribute nothing to facial contour. Swelling in the first few days makes the cheek look fuller temporarily, which is where this idea comes from, but the underlying bone and jawline are unchanged once healing is complete.

Will I need stitches, and when do they come out?

Simple extractions usually need none. Surgical removals generally do, and most practices use dissolving stitches that disappear within one to two weeks on their own. If non-dissolving stitches are placed, removal is a quick appointment about a week later and does not require anesthetic.

What if I cannot open my mouth fully?

Limited opening is common when the gum over a wisdom tooth is infected, and it usually eases once the infection settles. Tell your dentist before the appointment, because severe restriction can affect access and may mean treating the infection first and removing the tooth at a second visit.

Can a wisdom tooth be removed while it is infected?

Sometimes, but not always. An actively spreading infection is often settled with antibiotics and drainage first, because anesthetic works less predictably in inflamed tissue. Your dentist will assess whether to proceed immediately or stage it. Antibiotics alone never resolve the underlying problem, they only buy time.
